背景:輪值三班常造成護理人員睡眠障礙,除影響精神狀態及生活品質,更可能發生意外事件。目的:探討耳穴按壓對護理人員睡眠品質、嗜睡、疲憊之改善成效。方法:對象為北部某醫學中心睡眠障礙之護理人員,研究工具為具信效度之匹茲堡睡眠品質、愛普沃斯嗜睡及疲憊量表,共收案88位,介入措施為耳穴按壓,於神門穴、心穴、皮質下穴、枕穴四個耳穴貼上減敏膠布,每日執行自我耳穴按壓3次,於耳穴按壓前及執行二週後、四週後的資料,比較執行前後之成效。結果:研究結果顯示睡眠品質耳穴按壓前平均分數得分9.32分,二週後下降為7.27分,四週後下降為6.44分,顯示耳穴按壓後睡眠品質有顯著改善(p<.001)。比較嗜睡平均分數由耳穴按壓前2.96分,二週後下降為2.30分,四週後下降為2.17分,顯示耳穴按壓後嗜睡程度有顯著改善(p<.001)。比較疲憊程度耳穴按壓前平均分數由7.50分,二週後下降為5.99分,四週後下降為5.75分,顯示耳穴按壓後疲憊程度有顯著改善(p<.001),皆達到統計上顯著差異。結論與應用:以減敏膠布貼於耳穴按壓對於睡眠品質、嗜睡及疲憊程度均有改善,未來可加以推廣,應用於其他有睡眠障礙之民眾或病人,提升其生活品質及健康。

Background: Nurses with shift work often suffer from insomnia. Poor sleep quality has a significant impact on working activity and quality of life, it also increases risks on nursing care leading to patient accidents. Purpose: This study was to explore the effectiveness of auricular acupressure on sleep quality, sleepiness, and fatigue level of nurses with sleep disorders. Methods: Subjects were nurses who suffered from sleep disturbance in a medical center, northern Taiwan. Pittsburgh Sleep Quality Index (PSQI), Epworth Sleepiness Scale (ESS) and Brief Fatigue Inventory (BFI) were used as instruments. Total 88 nurses completed this study. Four ear acupuncture points, including shen- men, heart, sub- cortex and occiput, were tagged with acne dressing. Participants performed self-administered ear acupressure trice every day. Data were collected in three time points at pre-acupressure, thereafter two weeks and four weeks post-acupressure intervention (PAI). Result: The average scores of the PSQI decreased from 9.32 (pre- acupressure) to 7.27 in PAI two weeks, 6.44 (p <.001) in PAI four weeks. The ESS scores decreased from 2.96 (pre- acupressure) to 2.30, in PAI two weeks, 2.17 (p <.001) in PAI four weeks. The BFI-Taiwan Form score decreased from 7.50 (pre- acupressure) to 5.99, in PAI two weeks, 5.75 (p <.001) in PAI four weeks. Conclusions: Auricular acupressure with non-irritating material improved sleep quality, and sleepiness, and reduced fatigue level of nurses. This study recommended that the investigated auricular acupressure might be applied in patients and people with sleep disturbance, to promote sleep quality and healthy.
