Objective: We aimed to study the melanocyte micro- and ultrastructure of the blue spot skin. Methods: The skin sample including the blue spot and non-blue spot region were taken from the 1 month, 3 months, 4 months, and 5 years old children. Light and transmission electron microscopy was used to examine the melanocyte structure of the blue spot and non-blue spot regions. Results: On a 1-month-old skin section, the melanocytes of the blue spot region at both the epidermis and dermis. From newborns to 3-4 months of age, melanocyte cells are evenly and densely distributed at all layers of the epidermis, especially the basal and the spinous layer, At the skin of a 5-year-old child, the cluster of melanosomes is located in the epidermis, but it is relatively less than that of a 3-, 4-months old children, and it is mostly located only in the basal layer. The cytoplasm of the melanocyte cell of the blue spot region is composed the several distinct types of melanin granules 2- one type has a light granule, and the other one has a dense type. In addition, the third alternative type of granules was found which contains a mixture of the light and dense types of the melanin granule. Conclusion: The melanocytes of the blue spot region were located at the both epidermis and dermis showing a slight movement from the dermal layer to the epidermal layers according to the age changing between 1 month to 5 years old. Several different types of melanin granules were found in the melanocyte cytoplasm of the blue spot region.
