緊急放射治療於腫瘤急症的適應症包括轉移腫瘤脊髓壓迫、上腔靜脈症候群、腦部轉移造成腦壓升高、腫瘤造成的大量出血等。轉移腫瘤脊髓壓迫是真正的放射治療急症，但若能先行手術減壓，對病人的神經學症狀改善幫助較大。上腔靜脈症候群極少造成立即生命危險，若病人狀況穩定，取得腫瘤病理切片並做完整的分期評估以決定治療方式是必要的。緊急放射治療的介入不僅需要放射治療團隊的協助，亦需要放射腫瘤科醫師的審慎評估及多專科的討論。
