咖啡是現代人常飲用的食品,其和骨質疏鬆的關係一直都有些爭議。咖啡影響到骨質的機轉,主要跟咖啡可能促進鈣從尿液中排出,以及減少腸胃道吸收鈣質有關。雖然經過上百篇的研究,此議題仍然沒有定論,但一般認為如果咖啡會增加骨質疏鬆的風險,多發生於攝取量超過每日4杯以上的停經後婦女,且在鈣質攝取量較低,和某些特殊基因型的族群風險較高。停經前婦女和非老年男性,即使是咖啡攝取量高,也許因為咖啡內其他成分和本身荷爾蒙的保護,比較看不到對骨質有特殊影響。雖然在臺灣,一般人鮮少攝取超過一日4杯的咖啡,且有在咖啡中加牛奶的習慣,但我們仍然建議咖啡愛好者,注意保持鈣質攝取量充足,增加日照和戶外活動,避免菸酒,以減少發生骨質疏鬆和骨折的機會。
