腰椎退化性疾病導致脊椎不穩定是五十歲以上族群常見的問題，症狀包括下背痛、下肢輻射狀疼痛與神經性跛行等，經過三到六個月的保守治療後，如症狀持續或是改善幅度有限的話，手術是有效的治療選項。手術的內容包含適度的神經減壓與內固定，內固定的方式經過多年的發展，目前大多數醫師合併使用椎弓骨釘(pedicle screw)達成的後外側固定(posterolateral fusion,PLF)與利用支架達成的椎體間固定(interbody fusion, IF)來達到穩定脊椎的效果，而良好的骨質則是內固定能不能順利達成的重要因素。
