體外震波(extracorporeal shock wave therapy)最早在1982年被應用於泌尿道結石的治療,之後逐漸應用於骨骼肌肉系統的疾病。體外震波乃針對慢性肌腱或韌帶疾病給予高能量震波,促進肌腱骨骼交界的血管新生、增加生長因子和蛋白的合成、促進細胞生長與分化,以達到組織再生與修復的療效,另外,震波也有止痛的效果,減緩病患的不適以期待改善生活品質。目前震波在骨骼肌肉系統治療主要是足底筋膜炎(plantar fascitis),肩關節鈣化性肌腱炎(calcified tendinitis)、肘關節肱骨上外髁肌腱炎(lateral epicondylitis)、阿基里斯肌腱病變(Achilles tendinopathy)、肌筋膜疼痛症(myofascial pain syndrome)等疾病,此外淋巴水腫(lymphedema)也可以用震波治療。目前臺北榮民總醫院復健部使用的體外震波為瑞士Storz Medical SD1,其配備有標準聚焦式、發散式震波探頭、與檢查用超音波探頭,在治療之前可以用超音波定位,並根據疾病的需求給予聚焦式震波或發散式震波,達到精準定位、有效治療。
