國內約有四分之一的民眾受胃食道逆流困擾，醫師除了給予良好的生活飲食衛教以外，降低胃酸分泌的藥物治療為國內主要治療，氫離子幫浦阻斷劑更是解決了大部分病患不適的症狀。然而過去在胃食道逆流治療上國內仍有四個臨床困境，包括長期用藥的安全及經濟考量、難治型胃食道逆流診治、食道酸鹼檢測及機能檢查的不普及和病患接受外科手術治療意願低。如果治療不佳也可能會有長期併發症包括食道狹窄以及巴瑞特食道病變、食道腺癌等等，更重要的是患者的生活品質以及工作低落增加身心疾病的風險。國外也相當重視胃食道逆流的診治議題，尤其是在精確的診斷以及個人化的治療上，當中包括了無線食道酸鹼膠囊(Bravo capsule)、食道阻抗性的測量以及內視鏡的微創治療等都是近十年來的重點，今年也特別發表了胃食道逆流診斷的里昂專家共識，提供醫師臨床很好的指引。國內自去年開始引進了無線食道酸鹼檢測膠囊以及熱射頻胃賁門緊縮術(Stretta procedure)兩項新利器，筆者所在的醫院也都開始提供相關診治服務。本文將簡介無線食道酸鹼檢測膠囊以及熱射頻胃賁門緊縮術的應用與療效。
