腫瘤治療的目的可分為根除性（radical）與緩和性（palliative）。對於緩和性腫瘤治療，放射治療是一種有效、耐受良好、且具有成本效益的方式。然而，治療目標為根除性或緩和性的界線有時不易區分，因此需根據病患相關因素（例：體能狀態、年齡、共病症），癌症相關因素（例：轉移性疾病、病理型態），與治療相關因素（例：之前接受的治療）等做判斷。治療計畫的設計，也需評估患者的存活時間，副作用的嚴重性，以及病患的運送與交通是否舒適與方便等，以達到緩和醫療最大的目標：增加病患生活品質。
