老年人難治型憂鬱症（treatment refractory depression, TRD）是一種常見的精神疾病，具有較高的合併症和死亡率。患有TRD的老年人通常有複雜的合併症和多種誘發危險因素，這可能導致神經精神功能障礙和治療反應不佳。目前有許多潛在的致病機制，包括：血管、免疫、老化或異常蛋白質沉積。TRD的治療策略包括：優化目前抗鬱藥物劑量、併用非抗鬱藥物、改用不同類別抗鬱藥物、並用不同類別的抗鬱藥物，以及非藥物輔助介入措施，例如腦刺激和心理治療。總之，老年人TRD的治療建議傾向於採用多模式方法，結合藥物和非藥物治療。
