急性下背痛的病因很多包括：肌肉及筋膜性疼痛、脊椎關節的疾病、薦髂或髖關節疼痛、腰椎的椎間盤突出、椎管狹窄等。下背痛只是臨床上的一個症狀，可能的原因非常多，常見包含軟組織受傷，腰與背部的肌腱或韌帶發炎；下背痛最常見也最常發生的部位通常是在第四、五節腰椎或第五節腰椎與第一節薦椎之間，俗稱脊椎骨刺（椎間盤突出症，herniation of intervertebral disc），診斷著重在詳細的病史詢問及理學檢查，確定疼痛點以及排除神經學上的危險異常訊號，必要時加上影像學檢查如X光，電腦斷層或核磁共振等。
