肩膀棘上出口切面攝影常用於診斷肩峰下骨刺或是旋轉肌腱環受傷所造成肩關節夾擊症候群,是顯示肩峰肱骨間隙的最佳投照法。本次研究是以自然站姿45°前斜位的攝影方法,來改善患者的配合度與方便性,進而降低肩膀棘上出口切面攝影影像的誤差率,以提高影像的品質。收集60 位肩關節不適患者,其中男性24 人,女性36 人,年齡35歲至88 歲,平均年齡62 歲,分成對照組與實驗組進行。對照組為傳統肩膀棘上出口切面攝影方式(身體轉45°~60°)做檢查,實驗組則是自然站姿45°肩膀棘上出口切面攝影攝影方式做檢查。所得到的結果,對照組病患的影像分數±SD 為3.50±0.94;實驗組病患的影像分數±SD 為4.17±0.38,兩組結果確實有明顯的差異性〈p < 0.05〉。本次的研究結果利用自然站姿45°來拍攝肩膀棘上出口切面攝影影像,確實避免病患因影像品質不佳而重新攝影,也能取得更有診斷價值的影像。

英文摘要

Shoulder outlet view is often used to diagnose shoulder impingement syndrome caused by subacromial spurs or rotator cuff tendon damage, and is the best projection method for the acromiohumoral space. This study adopted a 45° anterior oblique position for shoulder outlet view in subjects standing with natural stance to improve the cooperativeness and convenience of patients. It also aimed to reduce the error rate of shoulder outlet view and improve the quality of images. Sixty patients with shoulder joint discomfort, 24 male and 36 female with the average of 62 (ranging from 35 to 88) years of age, were recruited and divided into control and experimental groups. Subjects in the control group received traditional shoulder outlet view with 45°-60° turning of the body, while those in the experimental group received 45° shoulder outlet view in natural stance. The image score of the control group was 3.50±0.94, and that of the experimental group was 4.17±0.38; the difference is statistically significant (p<0.05). The results of the present study showed that by adopting a 45° anterior oblique position for shoulder outlet view in subjects standing with natural stance can prevent having to retake the image due to poor image quality, and obtain images with better diagnostic value.
