背景︰近年來,世界各地的氣喘盛行率有逐年上升的趨勢,氣喘若是嚴重發作時,可能產生呼吸困難的症狀,甚至死亡。目前已有研究指出飲食與呼吸道發炎之間的關係,但目前維生素D與氣喘的關係尚未明確。有研究指出,miRNA會參與調節發炎反應。因此本研究以流行病學的研究設計來探討維生維D、mir-21、mir-155與氣喘之間的相關性。 材料與方法︰本研究招募114位成人氣喘病患及114位社區健康對照組經由年齡、性別配對,收集問卷、肺功能及血液採集,用於檢測維生素D濃度。之後再排除沒有RNA檢體的研究對象,再一次年齡、性別配對,最後氣喘組及對照組各配對41人,用於檢測microRNA-21、microRNA-155基因表達。 結果︰本研究中氣喘組與對照組的維生素D濃度,兩組間並未達顯著差異。mir-155、mir-21在兩組間平均表現量也未達顯著差異。但基因表達若高於75%,其mir-155、mir-21兩組的人數皆有顯著差異,p值分別為0.04、0.002。在校正年齡、性別、腰臀比、IgE後,mir-155、mir-21基因表達≥75%者,罹患氣喘的風險皆有顯著上升,尤其是mir-21,其風險更是高達5.64倍(95% C.I. 1.451-21.929)。而在相關分析發現維生素D與肺功能FEV1/FVC達顯著相關,但未發現與mir-155、mir-21之間的相關性。 結論︰本研究發現其mir-155、mir-21的基因表現若是有高度表現,其罹患氣喘的風險會顯著上升,未來可以成為治療氣喘的標記物。但在維生素D與microRNA,則沒發現之間的相關性。 關鍵字︰氣喘、維生素D、MicroRNA-155、MicroRNA-21

英文摘要

Background︰ In recent years, the prevalence of asthma is an increasing trend around the world. When asthma was exacerbated, it may cause dyspnea, cough and even death. It has been demonstrated that the relationship between diet and respiratory inflammation, but the direction of association isn’t clear. Previous studies have demonstrated that microRNA was involved in regulating airway inflammation. Therefore, we designed a case-control study to validate the association between vitamin D, microRNA-155, microRNA-21 and asthma. Methods︰ We recruited 114 adult asthmatics and 114 age and sex- matched community controls in the present study, then used a structured questionnaire, tested for lung function, and collected blood samples to detect vitamin D levels. After excluding subjects without RNA samples, we matched 41 case control pairs for testing microRNA-155 and microRNA-21 gene expression. Result︰ No association was found between vitamin D concentrations and asthma(p=0.617). There are no significant between means levels of mir-155, mir-21 and asthma.The frequency of case group is significant more than control group for mir-155 and mir-21 with high gene expression (higher than 75% levels). In adjusting for age and sex, subjects with high gene expression of mir-155 and mir-21 significant increase the risk of suffering from asthma, especially mir-21, the risk is as high as 5.64 fold (95% C.I. 1.451-21.929). Furthermore, the association between vitamin D and FEV1/FVC was statistical significant. No correlation was found between vitamin D concentrations and mir-155, mir-21. Conclusion︰In our study, we found that high gene expression of mir-155 and mir-21 significant increase the risk of suffering from asthma. With regulating gene expression to treat asthma can reduce the use of medical resources in the future. We did not find the relationship between vitamin D and asthma, we also did not find the correlation between vitamin D and microRNA. Keywords︰asthma, vitamin D, microRNA-155, microRNA-21
