腹膜透析為居家治療方式,其中全自動腹膜透析機採智慧醫療,醫護人員藉由雲端取得治療數據,發現異常事件,讓病人獲得及時協助與建立管理模式。系統以黃旗、紅旗代表特殊或異常事件,使用後發現無法完全辨識特殊事件,且病人撥打值班電話比例仍偏高,我們改以醫療風險管理模式及參考醫療失效模式與效應,重新定義警訊事件舉旗設定。修正舉旗警示設定後,警訊較明確,有效率地監測病人透析狀況,≥1次警示事件的病人數由2022年60.5%及50.7%在2023年同期降為42.8%和36.2%,且值班緊急電話的使用率由更改定義前44.8%減少至33.3%,定期檢討警示設定,能減少護理疲乏,提升照護腹膜透析管理效能。 |
英文摘要 |
Automated peritoneal dialysis utilizes an intelligent medical approach. Medical personnel access treatment data through the cloud, enabling them to identify abnormal occurrences promptly and establish a management framework. The system employs yellow and red flags to signify special or unusual events. However, after its initial use, it was evident that certain events were not being accurately identified, resulting in a higher volume of patients calling the onduty phone number. Following the adjustment of the warning flag setting, the warning signal became more distinct, facilitating efficient monitoring of patient's dialysis status. This led to a reduction in the frequency of emergency calls from 44.8% in January 2022 to 33.3% in January 2023. Moreover, the percentage of patients experiencing more than one emergent event per month decreased from 60.5% and 50.7% in 2022 to 42.8% and 36.2% in 2023. These improvements allowed for early detection of abnormal patient events and enhanced the effectiveness of peritoneal dialysis care and management. |
