Peri-implantitis Induced by Residual Root Attached Onto Implant-A Case Report

Dental implant therapy is a common reconstruction method of missing tooth or teeth, thus, the complications associated with dental implant-supported crown/bridge increase gradually. A 64 years old female suffered from gum swelling over right lower area for years. She was referred from local dental clinic because of peri-implant irregular hard tissue shown in dental radiographs. The initial impression was foreign body around implant induced inflammation. The unknown hard body was taken out after removal of screw-typed crown; then, the decontamination of implant surface by water Laser and grafting with porire bone graft were performed during open flap procedure. The final pathologic report revealed "a fragment of dentine". The clinical symptom and sign disappeared after surgery, and original crown was re-tightened after 3 months. It was challenging to diagnosis this dental implant-related complication at first. However, to date, the implant-supported crown has functioned well without any clinical symptom and sign.

人工植牙治療是常見重建牙齒缺失的處理方法，因此，與人工植牙支撐的牙冠／牙橋相關的併發症逐漸增加。一名64歲的女性多年來斷斷續續地患有右下部牙齦腫脹。因為牙科X光片顯示種植體周圍不規則硬組織而轉診至本院，最初的診斷是人工植牙周圍異物引起的炎症。在翻瓣手術過程前，先取下螺絲式牙冠後再取出未知的硬體；然後通過鐳射對人工植牙表面進行去汙，最後將豬的人工骨粉充填入骨缺損處。最終的病理報告顯示為牙本質碎片。術後臨床癥狀和體徵消失，經過3個月的追蹤觀察後將原冠重新置入。一開始要正確診斷這個與人工植牙相關的併發症的病例具有挑戰性。幸好直到今天，種植體周圍缺損的治療結果還算良好。
