隨老年人口增長,中、老年人聽力障礙人口數也將會呈現等比例增加,助聽輔具使用狀況關係著溝通品質及生理與心理的健康。問卷調查五十歲以上助聽器使用者,問卷設計主要為兩部份:第一是受試者及輔具基本資料,包括:受試者基本資料、生活居住、輔具使用相關資料、保養維修等選項;第二為配戴後生活上的滿意度,問卷參考「簡短助聽器滿意度簡化量表」(APHAB)設計,為方便年長者作答,修改原有量表的七點更改為五點量尺,分析問卷各項結果及其關聯性。一百零五份有效回收問卷,受試者平均年齡為76歲,81.90%的個案領有殘障手冊;銀髮助聽器使用者當中59.00%是與子女同住者;41.90%是雙耳配戴助聽器;助聽器配戴類型以耳掛式助聽器佔了50.50%最高;每日平均配戴的時間約為7小時。助聽器取得41.90%是經由醫師或聽力師轉介。整體滿意度結果有41.90%尚可接受目前的助聽器,滿意目前所使用的助聽器有39%;不滿意有11.40%,主要因為吵雜問題。維修調查的結果有26.70%滿意度是肯定的,4.80%對於維修不滿意。生活上的滿意度結果顯示雖然不會因為噪音的存在而心有不耐,但其與人溝通時易受到噪音的影響,且與「安靜環境中的溝通情形」、「噪音感受度」間的差異達到顯著。本研究結果可提供專業相關人員對中、老年人聽力損失者之助聽器使用與滿意度參考,其中有近四成受試者感到滿意,四成受試者尚可接受,一成受試者不滿意,可見有相當大的改善空間,未來宜發展出多元化的主客觀評估工具,配合有系統的復健與追蹤計畫,才能有效協助中、老年人聽力損失人士。

英文摘要

Background and purpose of research: The hearing impaired population in elderly will increase as a ratio by a growing up aging group. The usage of hearing aids could be related with their communication quality and mental health. Therefore, the types of the hearing aids, the aided condition, the fitting procedures, the information of purchasing, the maintaining of hearing aids and the entirety of the satisfaction rate in elderly persons who had fitted hearing aids will be investigated. The goal of this research is to assist to improve the process of hearing aid fitting and the clinically hearing assessment in elderly. Methods: Hearing loss subjects who are hearing aid wears and age above 50 years old. The questionnaires are mainly consist of hearing aid wears data and modified abbreviated profile of hearing aid benefit (APHAB) in Chinese. The answer items of APHAB had been revised from seven to five scales for elderly people choose out easily. Results: Sixty-seven male and thirty-eight female subjects participate this study. Eighty-one per cent of the participants were received a part pension for hearing aids. Forty-two per cent of the subjects aided bilaterally. Fiftyone per cent of the participants who were aided the behind the ear, and thirty-eight per cent of those were aided in the ear. The daily use of the hearing aid of all participants was 7 hours in average. Most of the participants (42%) who had been referred to fit hearing aid by doctors and audiologists. Forty-two per cent of subjects were fairly satisfied their hearing aids. The most reason which they had complained problem was noisy. The result of APHAB indicated that elderly hearing aid users could tolerate the environment noise but could not in conversation with people. Conclusion: Hearing services in elderly need to accommodate many considerations. Developing the diagnostic programs for elderly hearing loss people will become an important issue. The professionals who dedicated in the hearing care field should not only provide hearing aids fitting but also administer to systemic follow-up and rehabilitation programs. The benefits of hearing aid will meet the needs of elderly hearing loss people and enhance their communication and life quality.
