注意力缺陷過動症是兒童最普遍的發展性神經行為障礙,患孩為兒童身心科門診的主要就診族群。本文分享一位10歲的注意力缺陷過動症男童,除了接受藥物治療,近半年加入規律運動及柔道訓練的輔助治療過程與照護經驗,護理人員除了用藥指導,亦配合患孩興趣鼓勵參加柔道訓練、注意運動安全、提供飲食衛教減輕藥物副作用及加強親師溝通之指導。本文個案在原有的藥物、修正親師教養方式、規律運動之外,輔以柔道訓練後,具體成效包括:情緒漸趨穩定、暴怒次數減少、體適能強化、專注力提升並能按時完成功課。此外,也因運動後食慾大增,而使個案免於methylphenidate對於食慾不振、發展抑制的副作用。柔道訓練於小學階段多為團體訓練,患孩因而學習人際相處,課業上能詢問隊友、師長、結交更多朋友,同儕關係亦精進。因此,綜合案例分析與文獻查證,建議門診護理人員面對注意力缺陷過動症兒童,可鼓勵規律運動,甚至參與柔道等團體活動,以改善核心症狀,促進身心健康,且能提供藥物、運動安全、營養支持等相關護理指導,以促進兒童之身心健康與安全。 |
英文摘要 |
Attention-deficit hyperactivity disorder (ADHD) is a common neurobehavioral disorder in children and usually is treated at outpatient clinics. This paper presents a 10-year-old boy diagnosed with ADHD who in addition to standard medication therapy also received adjunct therapy for a period of 6-month regular exercise and Judo training. Regular exercise assists ADHD children in cerebral development and also increases cardiac stroke volume and cerebral blood volume. As a result of improvement in cognitive development, excretion of endorphin, decreased anxiety, stress and other psychological distress, it helped to relieve the central syndromes of ADHD in children. Judo training provides children with not only general physical exercise, but also promotes sensory integration. Judo training is a group training model that promotes the peer companionship that was missing in many of the ADHD children. This case study and literature review suggested that the neurobehavioral outpatient clinics may promote regular exercise and Judo training for ADHD children. |
