實證引發護理專業進行著令人矚目的發展和變革，已改變護理人員的思維與決策方式。實證科學是護理專業發展的必經之路，護理改革的推動者，需要具備滿足實現病人護理新願景的知識和方法，更要具備一個基於理論知識的框架和策略，才能朝著可預測，有效益，高效率，可靠，公平，以病人為中心的實證醫療照護體系前進發展。本文主要目的是介紹美國醫療照護改善研究機構的改進科學模式與澳洲喬安納布吉斯機構的證據臨床應用系統，系統性引導臨床工作者將實證措施導入臨床作業，此兩個系統可以提供護理師進行實證臨床應用時之參考。

A remarkable advancement in the nursing profession is the development and transformation of evi¬dence-based practice, which changed the way of thinking and decision-making of nursing staff. Evidence translation science is an essential pathway for this transformation. The promoters of nursing reform on the new vision of patient care need to posse both theoretical knowledge and change methods built on the frame¬work and strategy toward a predictable, effective, efficiency, reliability, and equity, of a patient-centered medical care system. The main purpose of this paper is to introduce the improved model from the Improve¬ment Healthcare Institute and the practical application of clinical evidence system from Australia, and sys¬tematically guide the nursing staff and leaders to proceed with the evidence translation processes.
