智慧型手機是現代人生活不可或缺的必需品，可內建之各式各樣應用程式如雨後春筍般不斷推層出新，其中包括各類的健康行動程式（Mobile health Application, mHealth App），也成了現今健康照護的新趨勢之一。對於肝臟移植的病人而言，肝臟移植手術延長了末期肝臟疾病病人的生命，而在生命延長的同時，更需要同步擁有良好的生活品質。移植術後病人需要服用多種藥物，維持良好的生活習慣，以避免發生感染或排斥的狀況發生，因此病人的健康自主管理能力，對於肝臟移植術後的生活品質有絕對影響。有鑒於此，本研究探討肝臟移植病人設計一肝移植專用之健康行動程式，探討使用此行動程式對於換肝術後病人之生活品質及健康自主管理的成效。統整相關文獻查證結果並與臨床現況進行分析後，與資訊工程師共同討論並設計一款針對肝臟移植病人專用Liver Transplantation App（LT-App），本研究以接受肝臟移植的病人作為收案對象，教導本App中各項功能及使用方法，並使用結構性問卷（生活品質、自主管理量表以及自我照顧認知測驗），分別在使用LT-App前、以及使用LT-App四週後蒐集資料並分析，藉以探討肝臟移植後LT-App介入對病人在健康自我管理之成效。期望藉此研究激發臨床護理人員，共同推動多元化之護理指導方式，提供更與時俱進的照護模式於肝移植術後病人，確實提升其健康相關生活品質。

Liver transplantation has prolonged the lives of patients with end-stage liver disease. However, patients need to not only extend their lives, but also have a good quality of life. The patient's ability to have independent health management has an absolute impact on the quality of life after liver transplantation. Smartphone mobile health programs are a new trend in health care today. Software in Chinese language for mobile health application (mhealth App) dedicated to liver transplantation patients was designed to include important items for self-management of daily life post transplantation. This study explores the effects of using this health mobile program before and after liver transplantation. A ＂Liver Transplant Patient Self-care Needs Questionnaire＂ based on data and evidence was designed to investigate the status of liver transplant patients during hospitalization in order to understand the patients' daily care needs. Through numerous discussions and revisions, the care team developed a Liver Transplantation App (LT-App) for liver transplant patients. The study subjects were patients post liver transplantations and collected the cases before using the App; use ＂Liver transplant patient health self-management assessment form＂ as a pretest. The nurse taught patients on the proper use and functions of the App. The study collected data using the same form four weeks after using the App as the post-tests. By the eighth week, patients were asked to provide a satisfaction survey on the use of this application. All data was collected and statistically analyzed to study the software application on the effectiveness on self-management and a health appraisal of post liver transplantation patients.
